
Followers of the Apocalypse are one of the game’s side factions, however, they can have a large impact on your playthrough as well as the ending you get. In this guide, I’ll explain how to easily max out your reputation with this faction.
Quick tips before you follow the guide:
- Spare medical items are required. To get the reputation, you’ll need a large number of Med-X, RadAway, and/or Fixer. You may either purchase these supplies from doctors and merchants around the world (including Julie herself), or you can find them.
- Complete High Times. While not required, completing the High Times sidequest given to you by Julie Farkas will allow you to pick up a single free Stimpack, RadAway, or Fixer per day. You can then turn in the Fixer or RadAway to her, so you don’t have to spend your own caps.
Increasing Your Reputation
First, make your way to the Old Mormon Fort, located in Freeside.


Enter through the main gate.

Find Julie Farkas, a tall woman with a mohawk.

Julie Farkas can usually be seen walking around the main courtyard. If you can’t find her, wait for a few in-game hours using the wait function.
Once you find her, talk to her.
She’ll ask you if you’re there to drop off medical supplies. Select the following dialogue options:
- “You need medical supplies?”

- “What do you need?”

- “You’ve got yourself a deal.”

- “Let’s talk about something else.”

- “I have medical supplies to drop off.”

Lastly, select whichever supplies you’re willing to donate, then donate them. You can donate up to 3 pieces of each medical item at a time, but you can repeat the donations infinitely until you become Idolized. You’ll have to leave the conversation for you to get an update on your reputation.
Depending on your Followers of the Apocalypse reputation when you start donating, it may take up to 20 donated items or more.
When you become Idolized, talk to Julie Farkas again. After you talk to her once or twice, she’ll invite you to join the Followers of the Apocalypse.

If you accept, she’ll give you a lab coat and a key to their safehouse.
